When User using Digital Signature Certificate on Employees Provident Fund (EPF) website and error displayed "Error: Name Provided in the form does not match with the Digital Signature Certificate. Retry" or as shown in the image.

Solution:
The name provided while registering the Digital Signature Certificate (DSC) on Employees Provident Fund Website is different from name given in the Digital Signature Certificate (DSC).

You are requested to check the name & Validity of user in the Digital Signature Certificate (DSC) as below:

Internet Explorer => Tools => Internet Options => Content => Certificates

Then, you are requested to type the name of user as per Digital Signature Certificate (DSC) while registering DSC and problem will be resolved.
